Description: The Addiction Severity Index survey application, designed for the Kentucky Drug Court program.

The adaptation of the Kentucky Drug Court Addiction Severity Index (KDC-ASI) was led by Dr. TK Logan in affiliation with the Center on Drug and Alcohol Research at the University of Kentucky. For more information about the development contact Dr. TK Logan.

The KDCASI online application is currently free of charge

File Description: The Adolescent Intake Assessment survey application, designed for the Kentucky Drug Court program.

The adaptation and development of the Kentucky Drug Court Adolescent Intake Assessment (KDC-AIA) was led by Dr. TK Logan in affiliation with the Center on Drug and Alcohol Research at the University of Kentucky. For more information about the development contact Dr. TK Logan.

The AIA application is free of charge. Use the link below to acquire the AIA application installer file. Use the "Save Link As..." or "Save Link to Disk" feature of your browser in order to download the files.

AIA currently works on Pentium-class machines or equivalent, with an operating system of Window 95, Windows 98, Windows ME, Windows NT and Windows 2000. The application has not been tested on Windows Vista.

Once the installation file is completely downloaded, run the installer by double-clicking on the file or selecting the file to be executed. Then follow the instructions by the installer.

Changes and adaptations can be accommodated upon request to Jeb Messer. A fee for adaptations or major changes may apply.
